Endoscopic & Percutaneous Microdiscectomy

Endoscopic & percutaneous microdiscectomy is a minimally invasive surgical procedure designed to relieve chronic back pain caused by herniated discs, nerve compression, or degenerative spine conditions.

Unlike traditional open spine surgery, this technique uses a small incision and specialized instruments, including an endoscope (a thin, flexible tube with a camera), to access and remove the portion of the damaged disc pressing on nearby nerves.

This minimally invasive procedure is one of the advanced treatments offered at Neuro Spine & Pain Center, designed to relieve pain and restore function with minimal downtime. Our spine and neuro pain specialists provide this cutting-edge treatment to help patients regain mobility and improve their quality of life.

Who is a Good Candidate For an Endoscopic and Percutaneous Microdiscectomy?

Endoscopic & percutaneous microdiscectomy is often recommended for patients who experience persistent back or leg pain (sciatica) that has not responded to conservative treatments. This procedure may also benefit those who have the following:

  • A herniated or bulging disc that compresses nearby nerves
  • Weakness, numbness, or tingling in the legs due to nerve compression
  • Difficulty standing, walking, or performing everyday activities because of pain

Before recommending this procedure, our Miami spine specialist will conduct a thorough evaluation, including imaging tests like MRI or CT scans, to determine if this approach is appropriate.

What are the Benefits of an Endoscopic and Percutaneous Microdiscectomy?

Compared to traditional open spine surgery, endoscopic & percutaneous microdiscectomy offers several advantages:

  • Minimally invasive approach: The procedure requires only a small incision, reducing tissue damage and scarring.
  • Faster recovery time: Patients typically experience a quicker return to normal activities compared to open surgery.
  • Reduced pain and discomfort: Less disruption to surrounding muscles and ligaments leads to a more comfortable recovery.
  • Lower risk of complications: Since the procedure is minimally invasive, there is a decreased risk of infection and other surgical complications.
  • Outpatient procedure: Many patients can go home the same day, avoiding the need for an extended hospital stay.

How is it Performed?

Endoscopic & percutaneous microdiscectomy is a state-of-the-art procedure performed under local or general anesthesia. The process involves the following steps:

  • The patient is carefully positioned, usually face down, on a specialized operating table. Fluoroscopy is used throughout the procedure to provide precise visualization of the spine and guide our surgeon.
  • A very small incision is made in the skin overlying the affected area of the spine. Through this incision, a thin, tubular retractor is inserted, creating a pathway to the herniated disc.
  • The endoscope, equipped with a camera and light source, is then gently advanced through the retractor. The endoscope projects a magnified, high-definition view of the surgical field onto a monitor, allowing the surgeon to visualize the nerves, ligaments, and disc with exceptional clarity.
  • Specialized percutaneous instruments are then introduced. These instruments are designed to be minimally invasive and allow for precise manipulation and removal of tissue.
  • Our surgeon will carefully remove the herniated portion of the disc that is compressing the nerve root.
  • After confirming nerve decompression, the instruments and endoscope are carefully removed. The small incision is typically closed with a stitch or surgical glue and covered with a sterile bandage.

The entire procedure typically takes between 30 to 60 minutes, depending on the complexity of the case. At Neuro Spine & Pain Center, our highly skilled surgeons experienced in performing the procedure utilize state-of-the-art technology to ensure precision, safety, and optimal outcomes for their patients.

What to Expect After an Endoscopic and Percutaneous Microdiscectomy?

Following the procedure, the patient is monitored in a recovery area. Pain is typically managed with oral pain medication. Once the patient is stable and comfortable and meets discharge criteria, they are allowed to go home, often within a few hours of the procedure.

During the first few days at home, it's important to rest and allow the body to heal. Light activities such as walking are encouraged to promote blood circulation, while strenuous activities should be avoided.

Most patients experience a significant reduction in leg pain (sciatica) almost immediately after surgery. Back pain may persist for a few days to weeks as the tissues around the surgical site heal.

Physical therapy typically begins within a week or two after surgery. Full recovery and return to all pre-operative activities, including sports and heavy lifting, usually take 6-8 weeks or longer. It's essential to gradually increase activity levels and listen to the body’s signals.

Scheduled follow-up appointments with our surgeon and Miami pain specialists are crucial to monitor healing progress, assess pain relief, and address any concerns.

What are the Risks and Complications?

Although endoscopic & percutaneous microdiscectomy is a safe and effective procedure. But like any surgical intervention, it carries some risks. These include:

  • Infection
  • Nerve damage
  • Recurrence of disc herniation
  • Persistent pain
  • Bleeding or blood clots

The likelihood of complications is low, especially when the procedure is performed by an experienced spine specialist and neurologist in Miami.
